Parent Permissions

Parent/Guardian named below gives permission for student(s)/children to participate in events and programs at West End Baptist Church, until cancelled in writing, and will allow student/children photos to be taken and used for church publications or social platforms. Parent/Guardian releases WEBC from all liability, claims or demands for personal injury, including sickness. Further, authorization is given to WEBC to furnish transportation and food related to church events, and to seek emergency medical care, including x-ray, anesthetic, or other medical care in the interest of the student. Parent/Guardian acknowledges that they are responsible for all expenses related to such emergency medical care.
